1. A money back guarantee request form, which will include your name, address and your reason for requesting a refund. Requests must be legible and the form must be completely filled out.  Forms with post office box addresses are not acceptable.
2. The money back guarantee request must be accompanied by your original sales receipt for the purchase of an Additech product.
3. The money back guarantee request must be postmarked within 60 days of the date of purchase on the original receipt.
4. Please mail your Money Back Guarantee Request Form
5. Money back guarantee requests are limited to one to each address.
